New Patient Information
For your first visit, we would like to welcome you to Mansell Family and Cosmetic Dentistry. You will meet Dr. Hepler and the entire staff.
For your convenience, you can complete the new patient information forms before your appointment through our website and bring them to your first appointment, or fill them out when you arrive. Please email or fax a copy of your dental insurance card before your appointment. For your first visit, please bring any other health or dental history information that may be helpful.
Your first visit will include a consultation with Dr. Hepler, complete dental x-rays, a comprehensive oral exam and periodontal exam, soft tissue exam and oral cancer screening, a treatment plan for health, and a healthy mouth cleaning, if no periodontal treatment is needed.
Financial Information
For your convenience, we accept cash, personal checks, VISA, Mastercard, and American Express.
For all dental treatment, you will be given a quote for the cost of treatment and asked to pay at the time of service. For those patients with insurance, we will calculate your estimated benefits and collect your portion at the time of service. We accept all types of insurance as long as your plan allows you to go to the dentist of your choice. We also will accept assignment of your insurance benefits saving you from having to pay for the full cost of treatment and wait for reimbursement from your insurance carrier.
